^a Since National Cancer Data Base (NCDB) data contain a sample of hospitals participating in the Commission on Cancer Accreditation Program, the percentages are based on the experience of hospitals located in these States, not based on patient residence.

^b The increase in the number of identified measure-eligible cases diagnosed post-2007 is attributable to changes in the Commission on Cancer Program Standards, which modified its requirements to mandate the reporting of American Joint Committee on Cancer clinical stage.

^c Includes data from Guam, the Virgin Islands, and other U.S. territories.
