a The measure includes all incident end stage renal disease patients in the given year who were under age 70 and had a known State of residence in the 50 States, the District of Columbia, or U.S. territories. The measure was calculated as the percentage (by Kaplan-Meier methodology) who received a living or deceased donor transplant within 3 years of their end stage renal disease initiation date.
