a Based on the eight Agency for Healthcare Research and Quality (AHRQ) Prevention Quality Indicators (PQIs) for angina, asthma, congestive heart failure, diabetes, and hypertension. For consistency of longitudinal reporting, the modified overall composite does not include AHRQ PQI 5 for chronic obstructive pulmonary disease because it is affected by International Classification of Diseases, 9th Revision (ICD-9-CM) coding changes.

b Rates are adjusted by age and gender using the total U.S. resident population for 2000 as the standard population.

State estimates are from the State Inpatient Databases (SID), and not all States participate in HCUP. Estimates for the total U.S. are from the Nationwide Inpatient Sample, which is drawn from the SID and weighted to give national estimates.
