Navigating the Primary Care Landscape through EvidenceNOW

EvidenceNOW: Advancing Heart Health was a multi-layered initiative operating in a complex primary care environment. Teams of practice facilitators and quality improvement specialists provided practice improvement support to primary care practices participating in EvidenceNOW. Additional support included expert consultation, shared learning collaboratives, health information technology support, and data, feedback and benchmarking to drive care improvements.
