This dashboard examines all reports on patient safety concerns submitted to the Patient Safety Organization Privacy Protection Center (PSOPPC) in Common Formats for Event Reporting–Hospital Versions 1.2 and 2.0. The Generic Dashboard details Annual Trend of Event Type by Report Type, Annual Trend of Report Type by Event Type, Location of Event by Event Type, Location of Event by Report Type, Distribution of Event Type Among Contributing Factor, Contributing Factors by Report Type, Harm by Event Type, Harm by Location of Event and Patient Age Category by Event Type. Selecting one of the options in the top table below will display a related figure and table. This information can also be downloaded as an Excel file from the links in the Additional Resources box. The total number of events in the Network for Patient Safety Databases (NPSD) from January 1, 2010, through December 31, 2025, is 3,766,017 events.
The NPSD does not contain a representative sample of patient safety concerns and cannot be used to calculate the actual incidence or prevalence of patient safety concerns. The reporting of patient safety concerns to the NPSD is voluntary as is the reporting to Patient Safety Organizations (PSOs) by providers. The NPSD is a summary of certain elements in Hospital Common Formats Events Reports for specific types of patient safety concerns that have been submitted voluntarily by a portion of the AHRQ-listed PSOs.
